Support

Support from a qualified nurse

You are not caring on your own: a qualified nurse is your main point of contact – from the first assessment to every reassessment. Your nurse plans the care with you, guides you and is there for you regularly.

  • A personal point of contact
  • A call every 2 weeks
  • A visit every month

Your point of contact

A professional at your side.

An experienced qualified nurse from Spitex Medimex supports you from the very first visit. They know your situation and are your main point of contact for questions about care and paperwork.

You provide the basic care; in the background, your nurse makes sure it is safe and professionally sound. This support is a fixed part of the employment: a Spitex must guide and supervise the relatives it employs – only then does the health insurer contribute to the basic care.

What your nurse does

  • Assesses your relative’s care needs in your home.
  • Plans the care together with you.
  • Guides you and shows you how to keep the care record.
  • Calls you regularly and visits you at home.
  • Answers your questions about care and paperwork.
  • Draws up an emergency plan with you.

The care process

Seven steps to safe care.

This is how your nurse plans the basic care with you – and how the care stays on track afterwards.

  1. Assess the care needs

Your nurse visits you and your relative at home and records what your relative needs help with.

  2. Make a nursing diagnosis

Using the interRAI HC Schweiz instrument, your nurse looks at the whole situation of your relative and their surroundings. Basic care needs no doctor’s order; the result is sent to the family doctor for information.

  3. Set care goals

Together with you and your relative, your nurse sets out what the care should achieve – personal goals that fit your daily life.

  4. Plan the care measures

The result is a care plan tailored to your relative: which basic care measures are needed and how much time is set aside for them. The plan is also the basis for your pay.

  5. Training and guidance

Your nurse trains you in care standards, hygiene and care quality, guides you through the measures and shows you how to record the care.

  6. Caring – with backup

You provide the basic care independently, following the care plan. Your nurse supports and guides you in the background.

  7. Reassess

Your nurse keeps checking whether the care is reaching its goals. At least every 6 months – sooner if needed – the needs are reassessed and the plan is adjusted.

Your rhythm

There for you, regularly.

Fixed intervals give you confidence and safeguard the quality of care. If more is needed, your nurse comes more often.

  1. Every day

    Care record

You briefly note down the care you have given. Your nurse shows you how.

  2. Every 2 weeks

    Phone check-in

Your nurse calls you at least every 2 weeks: how are you both doing, and what do you need?

  3. Every month

    Home visit

At least once a month, your nurse assesses the situation in person – more often if needed.

  4. Every 6 months

    Reassessment

The care needs are reassessed after 6 months at the latest – sooner if something changes.

The cantons of Zurich and Aargau also require calls and visits at this frequency.

Safety in daily life

Care with confidence – and take a breather.

Guidance, an emergency plan and – where possible – relief give you a firm footing in everyday caregiving.

  • Training and course

You do not need a nursing diploma. Your nurse guides you until you feel confident. In the cantons of Zurich and Aargau, you also complete a recognised care assistant course within your first 12 months. Spitex Medimex pays for it up front and covers half of the cost; your share is deducted from your wage in agreed monthly amounts. In the other cantons, we discuss with you whether a course makes sense for you – the same cost split applies.

  • Emergency plan

Together with your nurse, you draw up an emergency plan. It sets out what to do in an emergency and whom to notify – so you do not have to stop and think in a difficult moment.

  • Relief

Do you need a break, or are you ill or going on holiday? Depending on the situation, Spitex Medimex staff can relieve you during that time. We plan with you early on whether and how this is possible.

Our goal

At home for as long as possible.

Your relative should be able to live independently in familiar surroundings for as long as possible. We work towards this together with you.

  • You care safely and soundly – with guidance and backup.
  • You recognise your own limits and possibilities and get support in good time.
  • The situation at home stays stable – for your relative’s wellbeing.
  • Employment and expert support strengthen the quality of care given by family members.

Reaching your limits? Tell your nurse – together we will look for a solution.

When things change

Needs change – and so does the care.

If your relative’s situation changes, we adapt the care. You do not have to wait for the next reassessment.

More or less basic care

We reassess the needs.

Does your relative need more help – or less? Let your nurse know. They reassess the situation and adjust the care plan. The paid hours may change as a result, up or down.
