You care for your relative at home – we employ you for the basic care: with pay of up to CHF 35.00 gross per hour – depending on the canton of the person you care for. You are also covered by social insurance and have a qualified nurse at your side.

  • You are paid for the basic care you provide

What counts is the basic care you provide – up to the time set in the assessment, not every hour you spend with your relative.

  • Depending on the canton

Your hourly rate depends on the canton where the person you care for lives. You will know your exact pay before you start – it is in your employment contract. We employ family caregivers in the cantons of Zurich, Aargau, Schwyz and St. Gallen.

  • Gross means before deductions

As for every employee, your social insurance contributions are deducted from your gross pay. Spitex Medimex pays the employer’s share.

  • Tax and other benefits

Your pay is earned income and is taxed. We are happy to look at how it affects benefits such as supplementary benefits, premium reductions or social assistance with you.

What is paid

Basic care is the basis of your pay.

You are paid for helping with the everyday tasks your relative can no longer manage alone because of illness, as defined in the Swiss Health Insurance Benefits Ordinance (Art. 7 para. 2 let. c no. 1 KLV).

What counts as basic care

  • Bandaging legs and putting on compression stockings
  • Making the bed and positioning
  • Exercises and help with moving
  • Preventing pressure sores and protecting the skin
  • Help with oral and personal hygiene
  • Help with dressing and undressing
  • Help with eating and drinking

What is not paid

  • Treatment care – for example preparing medication, dressing wounds, giving injections
  • Companionship and support
  • Housework
  • Simply being present

If your relative needs treatment care, our qualified nurses take care of it.

How your hours are set

  1. Assessment at your home

A qualified nurse from Spitex Medimex assesses the care needs with you and your relative – using the interRAI HC Schweiz tool.

  2. The time a professional would need

The nurse sets how much time basic care takes – as it would for an outside carer. What spouses and family members owe each other anyway cannot be billed.

  3. No doctor’s order needed

Basic care does not need a doctor’s order. We send the result of the assessment to your family doctor for information.

  4. Reassessed regularly

At least every 6 months, earlier if needed. If the care needs change, the paid hours change too.

Social security

Insured while you care.

Caring for a relative unpaid often leaves you poorly protected. With this job, you are insured like any other employee.

If you care unpaid

  • You receive no pay for your care work – and no daily allowance after an accident either.
  • If you cut back at work to provide care, you pay less into the AHV and your pension fund. This can reduce your old-age pension and any disability pension.

Employed by Spitex Medimex

  • AHV/IV/EO: old-age, survivors’ and disability insurance and income compensation – your contributions count towards your future pension.
  • Unemployment insurance: you pay in, as in any job.
  • Accident insurance: occupational accidents are always covered, non-occupational accidents from 8 hours a week with us.
  • Pension fund (BVG): from an annual salary of CHF 22’680 (2026).

If your annual salary is lower, we will check with you whether you can still join our pension fund.

If you have already reached the AHV reference age, different rules apply – for example no unemployment insurance and no obligation to join a pension fund. We will discuss this with you.

Job and training

Caring, working, learning.

You can keep working in your job – and you learn what you need for basic care along the way.

Alongside your job

The employment can be combined with an existing job. What matters is that the two jobs are compatible under employment law – this is one of the requirements. We will look at this with you in the free consultation.

Do you meet the requirements?

Introduction and care assistant course

You do not need a nursing diploma: your nurse introduces you to basic care. In the cantons of Zurich and Aargau, you also take a recognised care assistant course within the first 12 months – these cantons require it. In the cantons of Schwyz and St. Gallen, we discuss with you whether you take a course; recognised courses are available there too, and the same cost split applies.

How we support you

We share the course costs

  • Spitex Medimex pays for the course up front and covers half of the cost.
  • You pay back your share in monthly amounts that we agree on together and deduct from your wage.

Good to know

Your rights and duties.

A job gives you security and comes with responsibility. So that you know what you are signing up for:

Your rights

  • An employment contract with Spitex Medimex and your pay every month for the basic care you provide – up to the amount set in the assessment.
  • Social insurance like every employee – pension fund and non-occupational accident cover from the legal thresholds.
  • An introduction to care and a qualified nurse at your side: they call you at least every 2 weeks and visit you at least once a month, more often if needed.
  • If you take a care assistant course, Spitex Medimex pays for it up front and covers half of the cost.
  • Information when the rules for employed family caregivers change.

Your duties

  • You provide the basic care set in the assessment. Treatment care is done by our professionals.
  • You document your care every day.
  • You take part in your nurse’s calls, visits and assessments.
  • If your relative’s condition changes, you tell your nurse.
  • In the cantons of Zurich and Aargau, you take a care assistant course within the first 12 months.
  • If you take a course, you pay back your share of the cost in monthly amounts that we agree on together and deduct from your wage.

The health insurer only pays for basic care if professional supervision is documented. That is why documentation, calls and visits are a fixed part of the job.

For your relative

What it means for your relative.

Your relative is cared for by Spitex Medimex – through you. So the same applies as with any Spitex.

  • Costs as with any Spitex

The deductible (franchise), co-payment (Selbstbehalt) and the cantonal patient contribution also apply when you provide the basic care. If your relative has not had Spitex before, this can mean new costs.

  • Assistance contribution and helplessness allowance

If your relative receives an IV assistance contribution, it is reduced by the time of basic care paid by the health insurer. A helplessness allowance is not reduced. We are happy to clarify this with you beforehand.

  • Treatment care from the same provider

Preparing medication, dressing wounds, giving injections: the qualified nurses of Spitex Medimex take care of it.
